Studying the intersection of Health and Homelessness
in Northwest Michigan
June 2019-January 2020
Through funding from the Michigan Department of Health and Human Services (DHHS), the Northern Michigan Community Health Innovation Region (CHIR) and the Northwest Michigan Coalition to End Homelessness (NWCEH) have embarked on 3 projects that will improve the coordination of services between health care providers and the homelessness service sector. The next phase of our work centers around identifying the places where hand-offs happen for people experiencing homelessness and health related systems. We take a look at "Catherine's" story to shine a light on the hand-offs she experienced during her housing instability:
